Italian restaurants in Saint-Petersburg are not so popular as in Western European cities. You won't find any little cozy places owned by a nice Italian family who offers good homemade food.
If you feel like Italian cuisine, then you will have to go to bigger, more fancy looking restaurants.
Il Patio has good Italian breakfasts, but I don't like there main courses. I find pizzas and pastas not so tasty. I didn't try everything there but my lasagna and pizzas of my friends were really bad. Plus the time between the order and the arrival of the dish to the table can be ridiculously long. Both times when I ended up in Il PAtio we had to finish the whole basket of baguette (at least it was good and fresh) until finally getting main dishes.
